Old School Essentials — Spell Reference Index

This is a form-fillable spell reference index designed for Old School Essentials, but which probably works with every OSR game.

It’s basically a spell book that can be used by any character class (clerics, druids, et seq).

It can be printed blank and filled in by hand, but its features are especially handy if you’re playing on VTT.

The first page of the sheet is an Entry Index. You can fill in the names and levels of your spells here, along with whether or not the spell is memorized. You can click on the entry number next to any spell and it will link you to the spell’s entry.

In the spell’s entry, you can add the spell’s range, duration, page citation, whether or not it’s reversible, and the spell’s description. In my personal games, I just copy and paste from the Player’s Tome but, obviously, I can’t make that a feature of the published sheet.

The upshot is that when you (or one of your players) wants to cast a spell, you don’t need to look everything up in the books. You can have one sheet with all your spells, see which are memorized, and pick one. Then click the entry number to see the spell’s information without having to look it up in the book.

Once you cast the spell, tick one of the “Memorized” check boxes off and it will remove it from the Entry Index on the first page. Then you can click the entry number again to take you back to the Entry Index, now updated to show your spell has been cast.

I’m attaching a screen shot from one of my players’ sheets to give you an idea how it works.

Let me know if it's buggy and I'll fix it. As always, it was designed with Acrobat and works best with Adobe, but should be fine with any pdf viewer.
